THE ASTHMA ACTION PLAN DIARY

The Asthma Action Plan Diary (out of stock and being udated) is an effective management tool for children with asthma. The Asthma Action Plan Diary aims to help children ages 7-13 to monitor their symptoms, peak flow, medication and triggers. The diary includes 5 sections:

  1. The Action Plan, completed by a doctor, helps the child and their family know what to do when symptoms improve or worsen.
  2. Signals, or symptoms of asthma, helps the child become more aware of symptoms. Monitoring daily symptoms, the child can use the action plan to ensure they are taking appropriate action.
  3. Medications include a list of the child's prescribed medications and a table for recording medications taken each day.
  4. Peak Flow includes space to record morning and evening peak flows. Using a graph, the child monitors peak flow patterns and can be altered to take action early.
  5. Notes is a section to record activities or exposure to triggers, enabling the child to understand the triggers affecting asthma.
